The address that is listed is the current address. Back in the day other address was right. I would keep it listed at 7028 Theatre Road. There would be less confusion in the location.

3609 Bardstown Road is correct. It is the corner of Bardstown Road and Hikes Lane. Both addresses (3623 and 3609) will both work.
