This theatre appears on the 1913 Sanborn, in an old wooden storefront that dated to sometime before 1886. By 1908, it had been expanded in the rear, and was used by a furniture and undertaker establishment.
This was likely a fairly basic conversion, and would not have survived competition with the large new theatre constructed to the west just a year later.
Later maps are not available online, but this was demolished long ago, and the current building dates to the 1960’s or 1970’s.

Note that the historical address was 124 E Broadway, near the corner with N Sweet. The streets were later renamed to the most boring system possible.
124 East Broadway is given as the address of a house called the Majestic Theatre, listed in the 1914-1915 American Motion Picture Directory. It was the only theater listed at Denison.
Thanks for the ID! It’s odd that the ‘new’ Germania wasn’t listed, since that opened in 1914.
