The Onalaska Area Historical Society notes the Crystal Theatre opened in June 1913, and in 1936, the building was converted to the Krause Dairy Bar by Edgar Brudos. Film Daily Year Book 1926 lists 300 seats.
In 1997 the La Crosse Tribune reported Dr. Michael Colvin was remodeling the building into his dental offices and today the name Crystal Theatre and 1913 is displayed with Midwest Dental on the front of the building.

Before acquiring its own building, the Crystal Theatre operated in a lodge hall, as noted in this item from the April 5, 1913 issue of Moving Picture World: “Onalaska, Wis.— The Crystal Co., which has been operating a moving picture theater in the Woodman Hall, has bought a site, formerly occupied by Thompson’s general store and will erect a picture play house.” The April 13 issue of MPW said that plans for the Crystal had been completed, but I haven’t found an opening announcement.
Uploaded two images from the Facebook pages of the Onalaska Area Historical Society. The double image shows the building as the Dairy Bar in 1959 and also how it appeared after the renovations by Dr. Colvin. The second is a 1919 advertisement for coming attractions.
