An image of the interior of the Star Theatre can be found in the August 31, 1910 issue of The Pawtucket Times, including an article, all on page 9. The image is posted in the photos section here. It appears the theatre had been refurbished and re-opened under new management at that time.
Confirmed on an 1895 atlas – the Masonic Hall was located just north of Leather Avenue on the west side of the street and is now part of the City Hall parking lot. It may have survived until urban renewal in 1968.

Here’s an interesting study of what was playing at the Star in 1912-13 and who was coming to see it on pages 95-96 of Americanizing the Movies and Movie-Mad Audiences, 1910-1914 by Richard Abel (University of California Press, 2006). Other references to Pawtucket theaters appear throughout the book.
Unless the house numbers were radically altered, this theater also no longer exists. 116 or 126 Roosevelt Avenue would be the parking lot for City Hall.
All the city directories I’ve consulted list the address as 116 North Main Street. North Main Street in Pawtucket later became known as Roosevelt Avenue.
